How can I find out what machine is causing the user account to get locked out after 3 attempts.

If your user is running Windows XP and they recently changed their password, you may want to look into a small bug I found in Windows XP's password management.
If the user is on a domain, we found that many had saved password information that conflicted with the new password. This saved password is not used in the login dialog, but is used first for any system scripts that require authentication to attach to resources (ie. mapping a drive).
Look in the Control Panel for User Accounts. If they are local Admin to the machine, you should have an advanced tab with a Manage Passwords button. If they are anything other than admin, they may be prompted for admin credentials. Cancel that (if needed) and there will be a link for them to manage their saved passwords.
In either case, remove any refferences to any local servers or resources form that list...
Once this is done, unlock the user's account, again, and it should stay that way.
This is all assuming there isn't actually another machine logged in somewhere locking the account...
